Job summary

Focused Post Acute Care Partners (FPACP) is seeking a Licensed Physical Therapist to evaluate patients and develop physical therapy plans in collaboration with physicians. The role involves administering active and passive therapies, instructing patients in activities of daily living, and supervising any Physical Therapy Assistant assigned.

The position reports to the Rehab Director, EDO, within the Therapy department.

Qualifications

• Bachelor's, Master's, or Doctorate in Physical Therapy from an accredited college or university.
• Current license/registration by state of hire as a Physical Therapist.

Responsibilities

• Establish, implement, and evaluate treatment plans with goals and discharge plans.
• Provide direct patient care to ensure effectiveness of PT programs.
• Apply modalities and therapies (heat, light, ultrasound, electrical stimulation, etc.) as prescribed.
• Coordinate with physicians and other clinicians to optimize patient care.
• Communicate plan of care and progress with patients, families, and care team.
• Supervise PT Assistants when assigned and participate in interdisciplinary conferences.
