The role requires BLS within 30 days, an approved PT license, and a Bachelor’s Degree in Physical Therapy. You’ll perform assessments, develop plans, document outcomes, and collaborate with referring providers while maintaining compliance

Qualifications

• Must hold a Bachelor’s Degree in Physical Therapy and a valid PT license.
• BLS certification within 30 days of start date.
• Knowledge of clinical documentation and treatment planning.

Responsibilities

• Designs an appropriate plan of care that integrates goals, treatment, outcomes, and discharge plan.
• Performs examination, selects reliable methods, and documents findings per policy.
• Educates patients and families using effective teaching methods while ensuring safety and ethical care.
• Communicates with referring providers on patient progress and recommendations.
