Job summary

Lincoln Meadows Care Center in Lincoln, CA is seeking a compassionate Physical Therapist to join our rehabilitation team. This full-time position offers hands-on patient care, collaborative planning, and a supportive environment where residents regain independence and quality of life.

What you’ll do: evaluate and treat residents, create individualized plans, document in Net Health or PointClickCare, work with an interdisciplinary team, educate families, and bring positive energy.

Qualifications

• Active CA Physical Therapist License or eligibility to apply.
• Graduate of an accredited Physical Therapy program.
• Experience with EHR systems such as Net Health or PointClickCare is a plus.
• Team player with strong communication skills.
• New grads welcome to apply.

Responsibilities

• Evaluating, creating individualized treatment plans and celebrating patient progress.
• Maintain accurate and timely documentation in accordance with facility guidelines.
• Help residents regain strength, mobility, confidence, and independence.
• Work alongside a supportive interdisciplinary team.
• Educate residents and families while building meaningful relationships.
• Bring positive energy and compassion to work every day.
