Job summary

Apple Rehab seeks a licensed Physical Therapist to provide patient-centered rehabilitation services on-site. The PT will evaluate conditions, design individualized treatment plans, and work with the team to maximize mobility and function for residents.

The role emphasizes adherence to safety, HIPAA, and teamwork with physicians and staff to deliver high-quality care in a local, family‑oriented environment.

Qualifications

• Graduate of an accredited PT program.
• Current PT license, in good standing with the State licensing board.
• Federal-, State- and GHR-required credentials current and on file.
• Cognitive skills as related to the position.

Responsibilities

• Plans and administers medically prescribed physical therapy treatment for patients.
• Reviews physician's referral and patient records to determine PT needs.
• Tests and measures strength, motor development, sensory perception, function.
• Plans and prepares written treatment programs based on evaluation.
• Administers manual exercises to improve and maintain function.
• Instructs and assists patients in activities and use of assistive devices.
• Records treatment, response, and progress in charts or systems.
• Confers with physicians to optimize treatment plans and integrate care.
• Orients and directs work activities of assistants and aides.
• Completes required orientation and adheres to safety and privacy protocols.
