Job summary

Apple Rehab in Connecticut is seeking a licensed Physical Therapist to evaluate patients, develop personalized rehabilitation plans and help restore maximum movement and function. You will work with a team to deliver evidence-guided therapy and coordinate care with physicians and staff.

Qualifications

• Graduate of an accredited PT program.
• Current PT license, in good standing with the State licensing board.

Responsibilities

• Plans and administers medically prescribed physical therapy treatment to restore function and relieve pain.
• Reviews physician's referral and patient condition to determine required therapy.
• Tests and measures strength, motor development, and functional capacity; records findings to revise treatment plans.
• Instructs and guides patients in exercises and use of devices; prepares written treatment programs.
• Confers with physicians and staff to coordinate care and integrate therapy with overall health care.
