Job summary

Smart IT Frame LLC in Elizabethtown, KY seeks a dedicated Licensed Pharmacist to join its Long-Term Care pharmacy team. You will review medication orders, supervise preparation, and collaborate with physicians and nursing staff to ensure safe, appropriate therapy for LTC residents.

The ideal candidate holds an active Kentucky Pharmacist license, has LTC experience or is willing to work in a fast-paced LTC setting, and demonstrates strong clinical judgment and communication skills.

Qualifications

• Active Pharmacist license with Kentucky Board of Pharmacy is required.
• LTC pharmacy experience preferred.
• Strong analytical and attention to detail in a fast-paced LTC setting.

Responsibilities

• Evaluate medication orders for dosage, interactions, and overall appropriateness.
• Supervise preparation, labeling, and dispensing of medications.
• Communicate with facility staff and physicians on medication-related questions.
• Review patient therapy and suggest changes to physicians or staff when needed.
• Maintain accurate patient databases and pharmacy documentation.
