Job summary

Lakes at Lutz Health and Rehabilitation Center in Florida seeks a compassionate Occupational Therapist (OT) to join our Skilled Nursing Facility rehabilitation team. This role evaluates residents, creates individualized treatment plans, and provides OT services to promote independence and safety in daily living.

The candidate will work within an interdisciplinary team, supervise COTAs as applicable, and ensure compliance with Medicare regulations.

Qualifications

• Master’s or Doctorate degree in Occupational Therapy from an accredited program.
• Current OT license in good standing in Florida.
• Experience in skilled nursing or long-term care preferred.
• Knowledge of geriatric rehabilitation and Medicare regulations.
• Strong communication and documentation skills.

Responsibilities

• Evaluate residents and develop individualized OT treatment plans.
• Provide therapy focused on ADLs, mobility, strength, coordination, cognition and safety.
• Monitor progress and adjust treatment plans as needed.
• Complete accurate documentation in compliance with Medicare and facility guidelines.
• Participate in interdisciplinary care and discharge planning.
• Supervise COTAs as applicable.
• Educate residents and families on adaptive techniques and equipment.
• Maintain compliance with federal, state, and facility regulations.
