Job summary

North Port Rehabilitation and Nursing Center in Florida is seeking a compassionate, licensed Occupational Therapist (OT) to join our skilled nursing facility rehabilitation team. The OT will evaluate residents, create individualized treatment plans, and provide therapy to promote independence, safety, and quality of life.

The role emphasizes ADLs, functional mobility, strength, cognition, and safety, with supervision of COTAs where applicable.

Qualifications

• Licensed OT in Florida in good standing.
• Knowledge of geriatric rehabilitation and Medicare regulations.
• Strong communication and documentation skills.
• Experience in skilled nursing or long-term care is preferred.

Responsibilities

• Evaluate residents and develop individualized OT treatment plans.
• Provide therapy focused on ADLs, functional mobility, strength, coordination, cognition, and safety.
• Monitor resident progress and adjust treatment plans as needed.
• Complete accurate and timely documentation in compliance with Medicare and facility guidelines.
• Participate in interdisciplinary care planning and discharge planning.
• Supervise COTAs as applicable.
• Educate residents and families on adaptive techniques and equipment.
• Maintain compliance with federal, state, and facility regulations.
