Job summary

Oak Haven Rehab and Nursing Center in Florida is seeking a licensed Occupational Therapist (OT) to join our Skilled Nursing Facility rehabilitation team. The OT evaluates residents, develops individualized treatment plans, and provides therapy to promote independence, safety, and quality of life.

The role involves ADLs, mobility, and cognition-focused therapy, progress monitoring, and Medicare-compliant documentation.

Qualifications

• Master’s or Doctorate in Occupational Therapy from an accredited program.
• Current OT license in good standing in Florida.
• Skilled nursing or long-term care experience preferred.
• Knowledge of geriatric rehabilitation and Medicare regulations.
• Strong communication and documentation skills.

Responsibilities

• Evaluate residents and develop individualized OT treatment plans.
• Provide therapy focused on ADLs, functional mobility, strength, coordination, cognition, and safety.
• Monitor resident progress and adjust treatment plans as needed.
• Complete accurate and timely documentation in compliance with Medicare and facility guidelines.
• Participate in interdisciplinary care planning and discharge planning.
• Supervise COTAs as applicable.
• Educate residents and families on adaptive techniques and equipment.
• Maintain compliance with federal, state, and facility regulations.
