Job summary

Santa Cruz Post Acute is seeking a compassionate Occupational Therapist (OT) to join our rehabilitation team in Santa Cruz, CA. The role supports residents to regain functional independence through evaluations, treatment plans, and targeted interventions within an interdisciplinary team.

Qualifications

• Strong interpersonal, communication, and documentation skills.
• Current California OT license (OTR/L).
• Graduate of an accredited Occupational Therapy program.

Responsibilities

• Perform comprehensive evaluations and develop individualized treatment plans.
• Provide direct therapeutic interventions to improve ADLs, mobility, strength, and independence.
• Document evaluations, treatment notes, progress reports, and discharge summaries.
• Collaborate with nursing, therapy staff, and physicians for coordinated care.
• Educate residents, caregivers, and families on safe techniques and home-care strategies.
• Participate in care conferences and contribute to interdisciplinary planning.
• Maintain compliance with state, federal, and professional guidelines.
