Qualifications

• Current, active OT license in Arizona (or ability to obtain)
• NBCOT certification required
• CPR/BLS certification required or willingness to obtain
• Experience in skilled nursing/post-acute care preferred
• Strong communication and clinical judgment required

Responsibilities

• Evaluate and develop individualized OT plans of care
• Deliver skilled interventions addressing ADLs, IADLs, cognitive and upper-extremity rehab
• Assess equipment and environmental modifications for safety and independence
• Supervise OTAs and rehab staff as required by state law
• Document progress and discharge outcomes according to regulations
