Mission Palms Post Acute Care in Mesa, AZ is seeking a Full Time Occupational Therapist to join our in-house rehab program. You will screen, assess, and treat patients with cognitive and ADL impairments and contribute to discharge planning.

Qualifications

• Licensed Occupational Therapist.

Responsibilities

• Screen/evaluate patients with cognitive and/or ADL disorders and develop plan of care.
• Use standardized assessments to support interventions.
• Provide rehabilitative, skilled treatment per regulatory standards.
• Administer therapies including ADL retraining, therapeutic activities, exercises, modalities, and assistive devices.
• Assess progress and adjust treatments in the clinical record.
• Evaluate outcomes and adjust services accordingly.
• Provide counseling to patients and families.
• Communicate with supervisor and health team members about progress and plans.
• Deliver comprehensive discharge summaries per requirements.
