Job summary

VHS Home Health Care in Virginia is seeking a licensed Occupational Therapist to provide skilled therapy across multiple counties including Newport News and Hampton. Full-time, part-time, and PRN opportunities available with competitive hourly pay of $45-$60.

You will assess patients’ abilities, develop personalized plans, and collaborate with families and medical teams to promote independence and safe daily living.

Qualifications

• A bachelor’s degree in Occupational Therapy from an accredited program is required.
• Current licensure or eligibility for licensure as an Occupational Therapist in the state of Virginia.
• Minimum of one (1) year of relevant clinical experience preferred.
• Strong ability to assess, analyze, and utilize clinical data to support effective decision-making and performance improvement.
• Excellent written and verbal communication skills.
• Proficient computer skills, including Microsoft Office and/or Google Suite platforms.

Responsibilities

• Assess patients’ physical, cognitive, and emotional abilities to identify challenges in performing activities of daily living.
• Develop and implement individualized treatment plans designed to improve independence, safety, and quality of life.
• Help patients relearn or adapt skills needed for daily living, including self-care, work, school, or leisure activities.
• Recommend, train, and educate patients on the use of adaptive equipment and assistive technology.
• Monitor patient progress and adjust treatment plans based on clinical findings and patient goals.
• Collaborate with families, physicians, nurses, and interdisciplinary team members to support optimal patient outcomes.
• Maintain accurate and timely clinical documentation in accordance with regulatory and facility standards.
• Participate in care planning meetings and contribute to discharge planning and continuity of care.
