Job summary

Pathways Healthcare is seeking a Home Health Aide to support patients in Nashua and surrounding areas. This part-time role runs Monday through Friday with flexible hours and pays $25.00 per hour. Service coverage includes Nashua, Merrimack, Londonderry and nearby communities.

We value compassionate, patient-centered care. You will assist with personal care and mobility, follow care plans, and document services for review by the supervising nurse.

Qualifications

• Licensed Nursing Assistant (NH preferred) or HHA certification accepted.
• Must be 18 years of age or older.
• Ability to read and follow written care plans and document services.
• Ability to work independently with minimal supervision.
• Compassion and empathy for ill, injured, or elderly.
• Current CPR certification required.
• Strong interpersonal skills, patience, and professionalism.
• Valid driver’s license and insured vehicle.
• Ability to assist patients with repositioning, transferring, and other physical tasks.

Responsibilities

• Provide personal care such as bathing, grooming, dressing, and mobility support.
• Observe and report changes in patients’ condition to the supervising nurse or therapist.
• Document care provided and patient observations accurately; follow the care plan.
• Maintain a safe and supportive home environment for patients.
• Promote quality, continuity, and safety of patient care.
