Job summary

Pathways Healthcare, a leading home health and hospice provider in New Hampshire, seeks a caring Home Health Aide to support patients in Manchester, Bedford, Hooksett, Auburn, Merrimack and Derry/Londonderry and surrounding areas.

Flexibility is key as shifts vary; candidates must have NH CNA or HHA certification, CPR, a valid driver’s license, and reliable vehicle to travel between patient homes. Join a mission-driven team delivering compassionate in-home care.

Qualifications

• CNA in NH preferred; HHA certification accepted.
• Must be 18 years or older.
• Ability to read and follow care plans and document services.
• Ability to work independently with minimal supervision.
• Compassion and empathy for ill, injured, or elderly.
• CPR certification required.
• Strong interpersonal skills and professionalism.
• Valid driver’s license and insured vehicle.
• Assist patients with repositioning and mobility.

Responsibilities

• Provide personal care such as bathing, grooming, dressing, and mobility support.
• Observe and report changes to supervising nurse or therapist.
• Document care provided and patient observations accurately.
• Follow the RN-established care plan.
• Maintain a safe, supportive home environment.
• Promote quality, continuity, and safety of care.

Job Description:

• Role: Home Health Aide
• Service Territory: Manchester, Bedford, Hooksett, Auburn, Merrimack, Derry/Londonderry and surrounding
• Schedule: Flexible

Qualifications:

• Licensed Nursing Assistant (CNA) in New Hampshire preferred ; Home Health Aide certification accepted
• Must be 18 years of age or older

• Ability to work independently with minimal supervision
• Compassion and empathy for individuals who are ill, injured, or elderly
• Current CPR certification required
• Strong interpersonal skills, patience, and professionalism
• Valid driver’s license and reliable insured vehicle
• Ability to assist patients with repositioning, transferring, and other physical tasks.

Responsibilities:

• Provide personal care assistance such as bathing, grooming, dressing, and mobility support
• Observe and report changes in patients’ condition to the supervising nurse or therapist
• Document care provided and patient observations accurately
• Follow the care plan established by the Registered Nurse
• Maintain a safe and supportive environment for patients in their homes
• Promote quality, continuity, and safety of patient care
