Pathways Healthcare is seeking a Home Health Aide to provide compassionate in-home care in Londonderry, NH and surrounding areas. The role supports patients with daily living activities, follows established care plans, and reports changes to supervising staff.

The position requires CNA NH preference or HHA certification, CPR, and reliable transportation. Flexible part-time hours and a patient-centered work environment are offered.

Qualifications

• CNA in NH preferred; HHA certification accepted
• Must be 18 years of age or older
• Ability to read and follow written care plans and document services provided
• CPR certification required
• Strong interpersonal skills, patience, and professionalism
• Valid driver’s license and reliable insured vehicle
• Ability to assist patients with repositioning, transferring, and other physical tasks

Responsabilités

• Provide personal care assistance such as bathing, grooming, dressing, and mobility support
• Observe and report changes in patients’ condition to the supervising nurse or therapist
• Document care provided and patient observations accuratelyFollow the care plan established by the Registered Nurse
• Maintain a safe and supportive environment for patients in their homes
• Promote quality, continuity, and safety of patient care

• Role: Home Health Aide


• Rate: $25.00/hour


• Main Office Location: Londonderry, NH


• Service Area: Nashua, Merrimack, Londonderry and surrounding areas


• Schedule: Monday - Friday (flexible)


• Job Type: Part-time

Qualifications:

• Licensed Nursing Assistant (CNA) in New Hampshire preferred; Home Health Aide certification accepted


• Must be 18 years of age or older


• Ability to read and follow written care plans and document services provided


• Ability to work independently with minimal supervision


• Compassion and empathy for individuals who are ill, injured, or elderly


• Current CPR certification required


• Strong interpersonal skills, patience, and professionalism


• Valid driver’s license and reliable insured vehicle


• Ability to assist patients with repositioning, transferring, and other physical tasks.
