Job summary

Fusion Healthcare Rehabilitation and Nursing Center is seeking an LNA to support residents with daily living activities, meals, hygiene, and mobility. The role emphasizes teamwork, accurate documentation, and following facility policies.

Qualifications

• LNA license is required.
• 1 year of LNA experience preferred.
• Experience with resident meals and hygiene is a plus.
• Ability to adhere to facility policies and standards.

Responsibilities

• Assist Residents with daily living activities, including meals and hydration.
• Support personal hygiene and mobility needs.
• Observe residents and report health concerns to the nurse on duty.
• Document resident food and fluid intake accurately.
• Maintain clean and orderly living areas and assist with transports as needed.
• Follow all facility policies and confidentiality requirements.

Duties and Responsibilities:

• Assist Residents with daily living activities, to include, delivering meals, turning and ambulating residents and providing plenty of fluids and nourishment meals throughout scheduled shift
• Help Residents with personal hygiene
• Observe Residents and report any health issues to the supervising nurse
• Document Resident food and fluid intake
• Assist Residents with feeding and serving nutritional supplements
• Making the Resident's beds and keeping their living areas clean and tidy
• Transport Residents within the facility
• Assist nurses and other staff as needed
• Adhere to professional standards and follow all facility policies and procedures
• Abide by all federal, state and local requirements and standards
• Maintain the confidentiality of all Residents and Facility data

Experience:

• LNA: 1 year (Preferred)
