Job summary

The LPN position at Covenant Living Communities and Services delivers patient care according to established policies. You will perform direct physical cares, document resident needs and coordinate with unit supervision, guiding non-licensed staff as needed.

You will monitor residents for safety and changes in condition, communicate with the charge nurse, assist admissions and discharges, and support families with explanations of care while maintaining confidentiality and compassion.

Qualifications

  • Requires high school diploma or equivalent plus at least 6 months full-time experience as a nursing assistant in a long-term geriatric setting.
  • Demonstrates leadership and cooperation to create a rehabilitative environment and growth.
  • Strong observation skills and ability to keep patient information confidential.
  • Well organized with compassion for senior adults and ability to communicate clearly in English.

Responsibilities

  • Deliver patient care according to established policies and procedures.
  • Document resident needs and conditions; supervise non-licensed staff on the unit when designated.
  • Communicate observations and concerns to the charge nurse and appropriate staff.
  • Monitor resident rooms for safety hazards and assist with mobility, lifting, and use of walkers/wheelchairs.
  • Assist admissions, orientation, discharge and transfers; help with scheduling and setup as needed.

Job description

We Are Inspired to Serve. Join us! This position is accountable for delivering patient care according to established policies and procedures. The position is responsible for carrying out direct physical cares, documentation of resident needs and conditions, and when indicated, direction of other non-licensed personnel on the nursing unit. This position may be designated as "responsible/ charge" on a unit on a particular shift, but is accountable to the LPN for monitoring and RN for supervision.

Communicate on a regular basis with the charge nurse regarding observations and suggestions of problem areas, resident and employee activities, concerns, needs, conditions, etc.

Observe residents' medical, mental and emotional conditions and communicate changes to appropriate staff. Provide explanation and offer understanding of nursing care to residents and families according to procedures and provide support relating to medical care as appropriate.

Monitors resident rooms for actual or potential safety hazards such as faulty electrical equipment, frayed electrical cords, scatter rugs, etc. and negotiates with the resident/family for replacement or removal. Demonstrates proper body mechanics in assisting residents, physical activities and lifting/moving items. Instructs/reinforces proper technique for residents in safe use of walkers and wheelchairs.

Assist in the admission, orientation discharge and transfer of residents including but not limited to: packing/unpacking belongings, placement of personal belongings, initiating needed work orders, orienting resident to the apartment and unit environment. Obtaining shower and laundry schedule for new residents, assisting with dining room seating and introductions.

  • 1 year experience as a Licensed Nursing Assistant
Knowledge, Skills And Ability
  • This position requires a minimum of a high school diploma or the equivalent plus a minimum of 6 months full time experience as a nursing assistant in a long-term geriatric setting.
  • Requires a spirit of cooperation and enthusiasm to create an atmosphere conducive to rehabilitation and growth. Proven leadership skills necessary.
  • Good observation skills are required. The position requires the ability to treat patient information confidentially.
  • The individual must be well organized and the ability to understand and show compassion to senior adults is essential.
  • Must be able to communicate (written and verbal) effectively in English with residents, family and staff.
Physical Requirements

The majority of time is spent between standing and walking. There is considerable amount of bending, stooping and supporting residents. Must be able to lift weights of 25 pounds, 50 pounds with assistance and the ability to support weights of 100 pounds. Manual dexterity and keen vision are necessary in use of equipment. May be exposed to unpleasant conditions and is at risk of exposure to potentially infectious material as part of the routine job functions.

Compensation Pay Range

$18.15 - $24.14 per hour

Reasonable Pay Estimate

A reasonable estimate of the pay range for this position is $18.15 - $24.14 per hour. There are numerous factors taken into consideration in determining the actual offered rate of pay, including but not limited to: job-related qualifications, experience, skills, education, geographic location, and consideration of internal and external equity.
