Job summary

Stillwater Creek Skilled Nursing & Therapy is seeking an Licensed Nurse (RN or LPN) to join our team in Stillwater, OK. This role supports a long-term care community and operates on a Monday-Friday 7a-7p schedule.

You will administer medications, monitor residents, and collaborate with families and physicians to ensure quality care. A current Oklahoma license and 1+ year of LTC experience are required.

Qualifications

• Current and unencumbered Oklahoma LPN or RN license.
• 1+ years of experience in a Skilled Nursing or Long Term Care environment.

Responsibilities

• Obtain and administer prescribed medications and treatments.
• Observe residents and record significant conditions; notify changes to relevant parties.
• Take vital signs and assess residents’ condition.
• Inform family and physician of changes in resident health.
• Modify care plans and related documentation with changes in condition.
• Prepare and administer injections and/or enemas.
• Collect samples and monitor catheters.
• Provide wound care.

Available Shifts:

• Monday-Friday 7a-7p

Responsibilities and Purpose

• Obtain and administer prescribed medications and treatment in accordance with approved nursing techniques.
• Observe residents and record significant conditions and reactions. Notify appropriate parties of residents’ change in condition and reaction to drugs, treatments and significant incidents.
• Take vital signs to detect deviations from normal and assess condition of residents.
• Keep residents’ family and physician informed of any changes in resident behavior, medications or conditions of health.
• Modify residents’ care plan, care directives, Activities of Daily Living (ADL) and Nurse Aide flow sheets in conjunction with residents’ changes in condition.
• Prepare and administer injections and/or enemas.
• Collect samples and monitor catheters.
• Provide wound care.

Required Experience and Education

• Current and unencumbered Oklahoma Licensed Practical Nursing (LPN) License OR Oklahoma Registered Nurse (RN) License.
• 1+ years of experience in a Skilled Nursing or Long Term Care environment.
