Enhabit Home Health & Hospice is seeking a licensed MSW to provide intermittent professional social work services to patients in home health or hospice settings in Kentucky. You will collaborate with physicians and care teams, assess patient needs, and educate families while following the plan of care.

A state license and at least one year of health care social work experience are required. This role emphasizes compassionate patient advocacy, care coordination, and eligibility for continuing

Qualificações

• Master's degree from CSWE-accredited school of social work.
• Must be certified or licensed in the state where they currently practice.
• At least one year of social work experience in a health care setting.
• Basic demonstrated technology skills, including operation of a mobile device.

Responsabilidades

• Administer social services to patients requiring intermittent professional services and teach the patient, family, and other members of the health care team.
• Perform services in accordance with the physician’s orders and the established plan of care (POC).
• Coordinate care within the home health/hospice team to ensure quality patient outcomes.
