Arista Recovery seeks a Licensed Mental Health Practitioner to provide counseling and case management for clients with substance use and co-occurring disorders. The role involves developing and monitoring individualized treatment plans, conducting assessments, and following up with therapy sessions to ensure progress toward goals.

You will liaise with nursing and operations, contribute to QA standards, and document treatment responses in the EMR system.

Qualifications

• Licensed state-wide mental health professional required or eligible (Psychologist/LPC/LCSW/LMFT)
• Bachelor’s Degree or higher in psychology, social work, counseling, nursing (clinical) or chemical dependency specialty preferred
• Minimum of two years’ experience in a mental health setting; substance abuse experience preferred
• Experience using an EMR system and contributing to treatment planning and progress notes

Responsabilités

• Provide clinical monitoring and coordinate program adherence to state and JCAHO standards
• Develop individualized, person-centered treatment plans with measurable goals
• Conduct follow-up therapy sessions aligned with treatment plans
• Complete admissions assessments and periodic re-assessments
• Serve as mental health liaison between nursing and operations
• Provide one-on-one interventions for clients with substance use and/or mental health issues
• Prepare monthly performance reports and participate in quality assurance activities
