Envision Health Inc. seeks a Licensed Professional Counselor to support a Medication-Assisted Treatment program.

The role involves conducting assessments, developing treatment plans, and providing individual and group counseling within a collaborative care model with physicians and care teams. Responsibilities include coordinating with medical staff, ensuring comprehensive documentation, and guiding recovery planning while addressing barriers to participation.

Qualifications

• Master’s degree in counseling, psychology, social work, or a closely related behavioral health field.
• Current and unrestricted LPC license in Virginia or approved credential.
• Experience providing counseling for substance use or co-occurring conditions.
• Knowledge of MAT, recovery-oriented care, relapse prevention, and trauma-informed planning.
• Ability to develop individualized treatment plans with measurable goals.
• Ability to document accurately and maintain required records.

Responsibilities

• Conduct comprehensive clinical assessments and substance-use evaluations.
• Develop individualized, measurable treatment plans with the treatment team.
• Provide individual, family, and group counseling for MAT clients.
• Coordinate with physicians, nurses, care coordinators, and peers for integrated care.
• Educate clients on MAT, relapse prevention, and recovery planning.
• Maintain HIPAA-compliant documentation and timely chart updates.
• Collaborate in discharge planning and transitions of care.
• Address barriers such as transportation, housing, and access to resources.
