Description
Family First Health is seeking a Part-Time Licensed Mental Health Professional with a desire to work with patients aged 5-100, for 20 hours/week in our Gettysburg and Hanover offices.

The Licensed Mental Health Professional works as part of a multidisciplinary Primary Care team, managing the overall health of primary care patients by offering brief (15-30 min), immediate behavioral health support and interventions to patients aged 5 and above. Responsibilities include assessment, evaluation, treatment, and appropriate referrals to specialty mental health services.
ESSENTIAL DUTIES AND RESPONSIBILITIES
- Provides targeted, evidence-based assessments and evaluations for behavioral health issues within the Primary Care setting.
- Utilizes the G.A.T.H.E.R model of care:
- Generalist: Works with patients of any age and behavioral concern.
- Accessible: Available to assist primary care physicians throughout the workday.
- Team-Based: Participates in medical meetings and huddles.
- Highly productive: Manages a high volume of patient care (8-10 patients/day; 30 min sessions).
- Education: Educates patients and supports the healthcare team regarding psychosocial needs.
- Routine: Integrates into daily workflow, destigmatizing behavioral health care.
- Provides brief, solution-focused interventions to promote well-being and support treatment plans.
- Offers skill-building, coaching, and psychoeducation.
- Conducts assessments to determine appropriate care levels and communicates recommendations.
- Provides consultation to patients and healthcare providers in a fast-paced environment.
- Uses screening and measurement tools to monitor patient responses.
- Provides advocacy and case management services.
- Maintains knowledge of community resources and coordinates referrals.
- Documents in electronic health records accurately, ensuring compliance with policies and regulations.
- Develops comprehensive, individualized treatment plans collaboratively.
- Assists in crisis situations following protocols, supporting trauma-informed care.
- Provides crisis intervention services to minimize crisis periods and acute care utilization.
- Requires travel between offices and occasional evening work.
- Adapts to workspace needs, including patient interactions in exam rooms and clinical areas.
- Meets productivity, documentation, and compliance standards.
- Participates in staff meetings, supervision, and training; completes annual training requirements.
- Performs other duties as assigned.
KNOWLEDGE/SKILLS/ABILITIES
- Active Licensed Clinical Social Worker (LCSW), Licensed Professional Counselor (LPC), or Marriage and Family Therapist (LMFT) in Pennsylvania.
- Current CPR and Narcan certification.
- At least 2 years of clinical experience.
- Experience in healthcare settings preferred.
- Bilingual (English/Spanish) preferred.
- Experience with Electronic Health Records (EHRs).
- Knowledge of Trauma-Informed Care, Motivational Interviewing, Recovery, and Illness Management practices.
- Proficiency in Microsoft Office and other software applications.
- Excellent communication, organizational, and interpersonal skills.
- Ability to work independently and as part of a team, handle sensitive information, and respond to crisis situations.

Working hours: Monday-Friday, 8:00 am – 6:00 pm, depending on office needs.
