Family First Health is seeking a Part-Time Licensed Mental Health Professional with a desire to work with patients 5-100, 20 hours/week position in our Gettysburg and Hanover offices.

The Licensed Mental Health Professional works as a member of a multidisciplinary Primary Care medical team, managing the whole health of Primary Care patients by offering brief (15-30 minutes), immediate behavioral health support and interventions to patients aged 5 and older. They provide assessment, evaluation, treatment, and referrals to specialty outpatient mental health services when appropriate.
Essential Duties And Responsibilities
- Provides targeted evidence-based assessments and evaluations for behavioral health issues within the Primary Care Setting.
- Utilizes the G.A.T.H.E.R model of care, which includes Generalist, Accessible, Team-Based, Highly productive, Education, and Routine aspects.
- Provides brief, point-of-care, solution-focused interventions to promote well-being and assist with treatment plans.
- Offers skill building, coaching, and psychoeducation to enhance overall health.
- Conducts assessments to determine appropriate care levels and provides feedback and recommendations to the Primary Care team.
- Provides consultation to patients, Primary Care Providers, and team members in a fast-paced environment, both formal and informal.
- Uses screening and measurement tools to monitor patient responses to care.
- Provides advocacy and case management services, maintains knowledge of community resources, and coordinates referrals.
- Documents and maintains electronic health records (EHR) in compliance with policies and regulations.
- Develops comprehensive, individualized treatment plans collaboratively with patients and providers.
- Assists in crisis situations following emergency protocols and coordinates with the treatment team.
- Provides crisis intervention services to minimize or eliminate periods of crisis and acute care utilization.
- Requires some travel between offices and occasional evening work.
- Demonstrates flexibility in workspace and patient engagement.
- Meets performance standards for productivity, documentation, and adherence to metrics.
- Adheres to compliance, laws, trainings, safety regulations, and participates in staff meetings and training.
- Performs other duties as assigned.
Knowledge/Skills/Abilities
- Active Licensed Clinical Social Worker (LCSW), Licensed Professional Counselor (LPC), or Marriage and Family Therapist (LMFT) in Pennsylvania.
- Current CPR and Narcan certification.
- At least 2 years of clinical experience.
- Experience in a healthcare setting preferred.
- Bilingual (English/Spanish) preferred.
- Experience with Electronic Health Records (EHRs).
- Knowledge of Trauma-Informed Care, Motivational Interviewing, Recovery, and Illness Management.
- Proficiency in computer software (Microsoft Office, others).
- Strong communication, organizational, and interpersonal skills.
- Ability to work independently and as part of a multidisciplinary team.
- Ability to handle sensitive information confidentially and respond appropriately in crisis situations.
- Ability to work efficiently with attention to detail.
- Ability to establish effective relationships with patients and team members.
- Adherence to all organizational policies, including Code of Conduct and Workplace Conduct.
- Ability to maintain required licensure and clearances.

Working Hours
Monday to Friday, between 8:00 am and 6:00 pm, depending on office needs.
