Jet Health Inc. is seeking a full-time Licensed Medical Social Worker (LMSW) to join our home health and hospice team serving the Boise, ID region and surrounding areas.

The LMSW will provide coordinated home-based care across age groups, performing psychosocial assessments, analyses, counseling, and referrals to meet patient and family needs. The role requires a Master's in Social Work with current LMSW/LCSW licensure, CPR certification, and at least one year of healthcare MSW experience.

Qualifications

• Master's or Doctoral Degree from an accredited school of Social Work.
• Active LMSW/LISCW/LCSW license in the state of practice.
• Minimum of one year MSW experience in a healthcare setting.
• Current CPR certification.
• Reliable transportation with a valid driver's license and auto liability insurance.

Responsibilities

• Perform psychosocial assessments for home health and hospice patients.
• Develop and evaluate the plan of care in collaboration with patient and caregivers.
• Provide counseling and education to patients, clients, and caregivers as ordered by the physician.
• Maintain accurate documentation of assessments, interventions, and communications.
• Collaborate with the interdisciplinary team to coordinate patient care.
• Participate in the agency's QAPI program and staff training.
• Stay updated on public and private eligibility standards.

First Choice Home Health & Hospice / Jet Health Inc. is currently seeking a full-time, Licensed Medical Social Worker (LMSW) to join our team to serve our patients currently enrolled in our home health and hospice services in the Boise, ID region and surrounding areas. The successful candidate will provide coordinated care in the home to patients of all age groups, performing psychosocial assessments, analysis, counseling, and referrals to meet the needs of the patient, client, and family. Requirements

• Conduct ongoing clinical assessments of home health and hospice patients to identify psychosocial, financial, environmental, and community resource needs.
• Develop and evaluate the plan of care in collaboration with the patient, client, representative (if any), and caregiver(s).
• Provide counseling and education to patients, clients, and caregivers as ordered by the physician and indicated in the plan of care.
• Maintain accurate and timely documentation of clinical assessments, interventions, and communication with other healthcare practitioners.
• Utilize effective interpersonal relations and communication skills to facilitate positive outcomes for patients and their families.
• Collaborate with the interdisciplinary team to promote coordination of patient care and ensure comprehensive support.
• Participate in the Agency's Quality Assessment and Performance Improvement (QAPI) program and in-service training.
• Stay updated on public and private eligibility standards and requirements, maintaining professional growth and competency.
• Promote the Agency's philosophy and administrative policies to ensure the delivery of quality care.

Requirements

• Master's or Doctoral Degree from an accredited school of Social Work.
• Active social work licensure (LMSW, LICSW, or LCSW) in the state of practice.
• Minimum of one year experience as a Master of Social Work (MSW) in a healthcare setting.
• Working knowledge of community resources and good interpersonal skills.
• Proof of current CPR certification.
• Reliable transportation with a valid driver's license and auto liability insurance.
