Job summary

Community Care Of West Virginia Inc. is seeking a licensed graduate social worker (LGSW) to meet the emotional and social needs of adolescents and adults, aligned with policy and standards. You will collaborate with the care team to plan, implement and evaluate psychosocial interventions and education.

The position involves direct clinical services under supervision, care coordination for high-risk patients, and comprehensive documentation while supporting CCWV's mission and values.

Qualifications

• Master's degree in social work (MSW) from a CSWE-accredited program.
• Licensed by the State Board of Social Work.
• Two years of supervised social work experience in a health care setting.
• Experience with adolescent populations and underserved communities preferred.

Responsibilities

• Engage patients and families in active care planning to address behavioral, clinical, social and environmental needs.
• Apply social work theory and methods, including assessment, planning, intervention and counseling.
• Conduct clinical or psychotherapeutic services under supervision, including individual, family and group work.
• Provide education and self-management support based on patient preferences.
• Provide care management to high-risk patients and clinical consultation to the care team.
• Maintain documentation for treatment goals, progress notes and billing.
• Promote teamwork, participate in committees, and support CCWV mission.
