Job summary

Community Care Of West Virginia Inc is seeking a licensed social worker (LGSW) to address the behavioral, clinical, social, and environmental needs of adolescents and adults. You will provide psychotherapeutic services under supervision and help develop treatment plans with families and treatment teams.

The role emphasizes education, care coordination, and documentation, with a focus on high-risk patients, interdisciplinary collaboration, and ongoing quality improvement within CCWV.

Qualifications

• Master degree in social work (MSW) from a CSWE-accredited program.
• Licensed by the State Board of Social Work.
• Two years of supervised social work experience in healthcare.
• Experience with adolescents and underserved populations preferred.
• C-CATODSW certification preferred.

Responsibilities

• Engage patients and families in care planning addressing behavioral, clinical, social, and environmental needs.
• Provide clinical or psychotherapeutic services under supervision, including individual, family, and group methods.
• Assess patient knowledge and provide education and self-management support.
• Deliver direct care management to high-risk patients and consult team members.
• Document treatment goals, progress notes, and billing according to standards.
• Promote cross-department collaboration and participate in committees as needed.
