Job summary

Qualifications

• Master's degree from CSWE-accredited program or PhD/DSW in social work.
• Licensed by the State Board of Social Work.
• C-CATODSW certification highly preferred.
• Ability to complete background check and fingerprinting.
• Experience with Medicare/Medicaid populations and telephonic engagement.

Responsibilities

• Provide psychosocial assessments and clinical diagnoses with treatment recommendations.
• Develop and monitor care plans with high-risk patients and families.
• Engage patients and caregivers in care planning, in person and telephonically.
• Coordinate with care team and link patients to community resources.
• Provide direct care and clinical supervision to LGSW/LCSW staff as needed.
• Document treatment goals, progress notes, and billing per standards.
