Job summary

PACS Group is hiring a Licensed Dietitian for a hybrid role primarily on-site in Perrysburg, OH. You will assess residents, develop care plans, and educate staff and families as part of an interdisciplinary team.

The position requires a current RD license, 2+ years in clinical nutrition (preferably long-term care), TPN/Clinimix experience, and strong documentation and communication abilities.

Qualifications

• Licensed Dietitian credential with current state licensure or eligibility.
• Minimum of two (2) years of clinical nutrition experience, preferably in long-term care.
• Experience managing Total Parenteral Nutrition (TPN) and Clinimix therapy for at least 2 years.
• Strong understanding of federal and state regulations related to long-term care nutrition services.
• Excellent assessment, documentation, and clinical decision-making skills.
• Excellent communication and collaboration skills.
• Ability to manage multiple priorities while maintaining resident care standards.

Responsibilities

• Complete nutritional initial, quarterly, annual, and significant change assessments according to regulations.
• Conduct monthly nutrition reviews for high-risk residents.
• Develop, implement, and monitor individualized nutrition care plans.
• Provide nutrition counseling and education to residents, families, and staff.
• Coordinate nutrition care services with the Dietary Supervisor and interdisciplinary team.
• Collaborate with physicians, nurses, and other professionals on resident needs.
• Monitor food service operations for regulatory compliance and quality.
