Job summary

PACS Group, Inc. in Perrysburg, Ohio, seeks a licensed dietitian (RD) for a hybrid role primarily on-site in our long-term care setting. You will assess, plan, and monitor nutrition care for residents, working with the interdisciplinary team to optimize outcomes.

Qualifications

• Licensed Dietitian with current state licensure or eligibility.
• Two years of hands-on experience with TPN and Clinimix therapy.
• Two years clinical nutrition experience, LTC experience preferred.
• Understanding of federal/state long-term care nutrition regulations.
• Strong assessment, documentation, and clinical decision-making.
• Excellent communication and collaboration skills.
• Ability to manage multiple priorities while maintaining resident care.

Responsibilities

• Complete nutritional assessments (initial, quarterly, annual, significant change) per regulations.
• Monthly nutrition reviews for high-risk residents (weight changes, pressure injuries, hemodialysis, tube feeding).
• Develop and monitor individualized nutrition care plans.
• Provide nutrition counseling to residents, families, and staff.
• Coordinate nutrition care with the Dietary Supervisor and care team.
• Collaborate with physicians and nursing staff on nutrition needs.
• Monitor food service operations for compliance with standards.
• Inspect diet trays for diet order compliance.
• Participate in daily interdisciplinary meetings and care planning.
• Maintain accurate, timely clinical documentation.
• Stay current with regulatory standards.
