Job summary

PACS is seeking a Licensed Dietitian to provide nutrition assessment, planning, and education in a predominantly on-site role with hybrid flexibility at our Perrysburg facility. You will work with an interdisciplinary team to optimize resident outcomes and ensure regulatory compliance.

Responsibilities include comprehensive assessments, care planning, monthly reviews for high-risk residents, and coordinated nutrition care with the dietary and clinical teams.

Qualifications

• Licensed Dietitian (RD) with current state licensure or eligibility.
• Minimum 2 years experience with TPN and Clinimix.
• Minimum 2 years clinical nutrition in long-term care preferred.
• Knowledge of federal and state regulations for long-term care nutrition.
• Strong assessment, documentation and clinical decision-making.
• Excellent communication and collaboration skills.
• Ability to manage multiple priorities while maintaining resident care.

Responsibilities

• Provide comprehensive nutrition assessment, planning, monitoring, and education for residents.
• Complete initial, quarterly, annual, and significant change assessments per regulations.
• Conduct monthly nutrition reviews for high-risk residents (weight changes, wounds, dialysis, tube feeding).
• Develop, implement, and monitor individualized nutrition care plans.
• Offer nutrition counseling to residents, families, and staff.
• Coordinate nutrition care with Dietary Supervisor and interdisciplinary team.
• Collaborate with physicians, nurses, dialysis dietitians, and others on resident needs.
• Monitor food service operations for regulatory compliance with safety and quality standards.
• Inspect diet trays for physician diet order compliance before meals.
• Participate in daily interdisciplinary meetings and care planning.
• Maintain accurate, timely clinical documentation and stay current with standards.
