Job summary

Rehabcentre in Perrysburg, OH is seeking a Licensed Dietitian (RD) to join our interdisciplinary healthcare team in a hybrid role that is primarily on-site for optimal resident care.

You will perform comprehensive nutrition assessments, develop care plans, provide counseling, and collaborate with nursing, physicians, and dietary staff to ensure regulatory compliance and high-quality nutrition services for residents.

Qualifications

• Licensed Dietitian with current state licensure or eligibility.
• Minimum of two years’ experience managing Total Parenteral Nutrition (TPN) and Clinimix therapy.
• Clinical nutrition experience in long-term care, skilled nursing, rehabilitation, or healthcare.
• Strong assessment, documentation, and clinical decision-making skills.
• Excellent communication and collaboration abilities.
• Ability to manage multiple priorities while maintaining high resident care.

Responsibilities

• Complete nutritional initial, quarterly, annual, and significant change assessments.
• Conduct monthly nutrition reviews for high-risk residents.
• Develop, implement, and monitor individualized nutrition care plans.
• Provide nutrition counseling and education to residents, families, and staff.
• Coordinate nutrition care services with the Dietary Supervisor and interdisciplinary care team.
• Collaborate with physicians, nurse practitioners, dialysis dietitians, nursing staff, and other healthcare professionals regarding resident nutritional needs.
• Monitor food service operations to ensure compliance with nutritional, sanitation, safety, quality, and regulatory standards.
• Evaluate food temperatures, portion control, preparation methods, presentation, and quality.
• Inspect diet trays for compliance with physician diet orders prior to meal delivery.
• Participate in daily interdisciplinary meetings and contribute to resident care planning.
• Maintain accurate, timely, and compliant clinical documentation.
• Stay current with federal, state, and professional practice standards.
