Equivalent combinations of education and experience that provide proficiency in the responsibilities may be substituted for the above requirements.

On-the-job physical activities include walking (1/3), sitting (2/3), talking or hearing (2/3), and using hands to finger, handle, or feel (2/3).
Position Summary: Collaborates with the interdisciplinary treatment team for developing and delivering treatment and discharge plans for palliative care patients. Performs patient assessments, emotional support, family meetings, bereavement support, and manages complex care transitions.
