Horizon Health Behavioral Health Services, a subsidiary of Universal Health Services, is seeking a Social Worker (LCSW/LCSW-A) for the PHP program in New Bern, NC. The role focuses on high-quality psychotherapy services across assessments, diagnoses, treatment planning, and discharge planning.

Responsibilities include individual, group, family therapy, and involvement in all three programs as needed, while maintaining ethical practice and active communication with the treatment team.

Formación

• Active LCSW or LCSW-A license in North Carolina with relevant experience.
• Minimum one year internship or experience in a similar position.

Responsabilidades

• Provide direct psychotherapy services including assessments, diagnoses, treatment planning, group/individual/family therapy, and discharge planning.
• Collaborate with multidisciplinary team and participate across all three programs as needed.
• Conduct timely progress notes and participate in CQI+ data reporting.
• Assist with discharge planning and connecting patients with community resources.
