Job summary

Cityblock is seeking a Licensed Clinical Social Worker (LCSW) – Behavioral Health to join our hybrid care model in North Carolina. The role involves triaging member needs, delivering short-term interventions, and coordinating referrals within a holistic care team.

The ideal candidate has at least one year post-master’s experience, holds an active LCSW license in NC, and will support members with SMI/SUD in both home-based and hub settings.

Qualifications

• 1 year post-master’s degree required.
• Active LCSW license or eligibility for licensure in NC.
• Experience serving members with serious mental illness or SUD is a plus.

Responsibilities

• Provide behavioral health support in-person and via telehealth.
• Develop member care plans when interventions are needed.
• Refer to internal/external providers for medical, social, behavioral health, and SUD services.
• Collaborate with care teams to determine escalation or placement needs.
• Maintain engagement with high-acuity members.
