Job summary

Horizon Health is seeking a licensed clinical social worker to join our behavioral health team in North Carolina. The role involves providing individual, family and group therapies across multiple programs, with coordinated discharge planning and ongoing patient education.

Applicants must hold an active NC LCSW/LCSW-A and a graduate degree, with at least one year of relevant internship or experience. Collaboration with nurses, physicians and therapists is essential to ensure quality care.

Qualifications

• Graduate degree required for clinical practice.
• Active LCSW or LCSW-A license in North Carolina.
• Minimum one year internship or experience in a similar position.
• Maintains up-to-date professional growth and compliance with training expectations.
• Knowledge of hospital and Horizon policies, safety standards and procedures.

Responsibilities

• Work in all 3 programs as needed and coordinate with supervisor.
• Conduct individual psychotherapy sessions per regulatory standards.
• Conduct family/group/activity/educational therapy sessions as directed.
• Provide ongoing education to patients and families.
• Engage in discharge planning with families, nursing staff and physicians.
• Complete progress notes and assessments per procedures.
• Participate in interdisciplinary treatment teams and CQI data reporting.

Education
