Job summary

FIRST CHOICE COMMUNITY HEALTH CENTERS INC is looking for a Licensed Clinical Social Worker (LCSW) based in Lillington, NC. The LCSW will provide individual, couple, family, and group therapy, assess patient needs, and create treatment plans.

Overview

SUMMARY: Licensed Clinical Social Worker (LCSW) serves as a part of a multidisciplinary team and provides direct services including individual (including youth), couple, family and group therapeutic services to patients, including assessments, treatment plans and discharge planning. LCSWs advocate and help clients gain access to resources while confronting their personal issues, such as mental illness, addiction, and abuse. LCSWs are expected to behave ethically and professionally, demonstrate excellent communication skills, and be capable of working with diverse populations. They must collaborate effectively with professionals from various social service and government agencies. The role requires emotional resilience and stability as the work often involves crises.

Responsibilities

• Addresses referrals from providers including referrals via warm hand-offs; consults with physicians, medical professionals and therapists when appropriate
• Participates in daily huddles to assess needs and coordinate services
• Maintains weekly scheduled appointments in our electronic records system (Greenway Intergy)
• Provides therapeutic services to patients and their families
• Collaborates with multidisciplinary team members including external partners
• Completes required documentation and paperwork including biopsychosocial assessments and treatment planning and implementation in a timely manner
• Demonstrates cultural competency and commitment to patients
• Demonstrates commitment to the philosophy of the corporation
• Maintains caseload of 15 to 20 patients weekly, with a minimum of 5 patients per day
• Provides emotional and mental health counseling to groups or individuals
• Builds strong rapport with clients and makes accurate assessments of their problems whether mental/emotional or physical in the case of substance abuse
• Keeps detailed and concise records of each patient during the course of their treatment
• Performs research tasks within a clinical setting
• Other duties as assigned

Supervisory Responsibility

The incumbent in this position does not have supervisory responsibility. However, they may be expected to provide leadership and instruction to volunteers and/or less experienced staff.

Qualifications / Position Requirements

Experience / Knowledge

• Working knowledge of clinic policies and procedures
• Licensed Clinical Social Worker (LCSW) from the North Carolina Board of Behavioral Sciences
• Knowledge and experience with evidence-based practices, including trauma-informed care framework
• Knowledge and experience working with co-occurring populations (chronic health conditions, mental health and substance use) with emphasis on children and adolescents
• Demonstrated oral/written skills. Strong clerical/administrative skills including personal computer skills (Microsoft Office software)
• Ability to make routine decisions and determine proper action based on experience within the limits of policy and procedures
• Ability to work effectively with people of diverse cultures, ages and economic backgrounds in a culturally competent and sensitive manner

Education

Master’s Degree in Social Work or related field from an accredited university. LCSW in good standing in the State of North Carolina with a minimum of two years post-licensure.
