Job summary

Chapa-De is seeking a licensed Behavioral Health Therapist (LCSW) to join our integrated care team in Auburn, CA. This full-time role provides initial evaluations, crisis intervention, brief therapy, care coordination, and referrals alongside primary care providers.

You'll work in a collaborative, community-focused environment with a strong emphasis on whole-person wellness. A sign-on bonus and relocation are available for eligible new hires.

Qualifications

• Licensed Clinical Social Worker (LCSW) required.
• Minimum of three (3) years’ experience in a Behavioral Health Program.
• Clinical experience in therapeutic settings with adults, families and children.
• BLS certification.

Responsibilities

• Provide initial patient evaluations, triage assessments, crisis intervention management, brief individual and group counseling, skills training, care coordination, case management and follow-up referrals.
• Participate in interdisciplinary case conferences, collaborate with primary care providers, and assist with program development.

Salary: $91,400- $106,050 full time annual salary.
