Concierge Home Care is hiring a Licensed Clinical Social Worker (LCSW) in the Panama City, FL area. You will deliver psychosocial support and behavioral health counseling to patients in their homes, developing care plans and coordinating with families and care teams.

Competenze

• Active, unencumbered Florida Licensed Clinical Social Worker (LCSW) license.
• Minimum of two years of clinical counseling experience.
• Home health or telehealth experience preferred.

Your Role as a Home Health Licensed Clinical Social Worker

As a Licensed Clinical Social Worker (LCSW), you will provide psychosocial support and behavioral health counseling to patients receiving care in their homes. You will play a vital role in addressing social, emotional, and environmental barriers that impact health outcomes.

Responsibilities include:

• Provide psychosocial assessments, counseling, and behavioral health support
• Develop and coordinate care plans to address social, emotional, and environmental needs
• Connect patients with community resources and support services
• Assist patients and families in navigating social, financial, and healthcare challenges
• Collaborate with physicians and interdisciplinary care teams to support patient outcomes
• Educate patients and caregivers on coping strategies and available resources
• Maintain accurate and timely documentation in accordance with Medicare guidelines
• Document visits efficiently using AI-assisted EMR tools

Qualifications

• Active, unencumbered Florida Licensed Clinical Social Worker (LCSW) license

• Valid driver’s license, reliable transportation, and auto insurance
