Job summary

Concierge Home Care in Pensacola, FL is seeking a Licensed Clinical Social Worker (LCSW) for a flexible, PRN role delivering psychosocial support in patients' homes. You will autonomously manage your caseload with AI-assisted charting and WellSky/Kinnser EMR tools, coordinating care with physicians and care teams.

Role emphasizes independence, meaningful patient connections, and opportunities for growth, while offering flexibility, competitive per-visit pay and mileage reimbursement.

Qualifications

• Active, unencumbered Florida LCSW license.
• Minimum two years of clinical counseling experience.
• Home health or telehealth experience preferred.
• Valid driver's license, reliable transportation, and auto insurance.

Responsibilities

• Provide psychosocial assessments, counseling, and behavioral health support.
• Develop and coordinate care plans to address social, emotional, and environmental needs.
• Connect patients with community resources and support services.

Your Role as a Home Health Licensed Clinical Social Worker
