Concierge Home Care in Spring Hill, FL is seeking a Licensed Clinical Social Worker (LCSW) to provide psychosocial support and behavioral health counseling to patients in their homes. You will address social, emotional, and environmental barriers affecting health outcomes.

You will conduct psycho-social assessments, develop care plans, connect families with community resources, collaborate with physicians and care teams, and document visits using AI-assisted EMR tools.

Qualifications

• Active Florida LCSW license.
• Minimum two years of clinical counseling experience.
• Home health or telehealth experience preferred.
• Valid driver’s license, reliable transportation, and auto insurance.
