Job summary

Mobile Medical in Pensacola, FL is seeking a Licensed Clinical Social Worker (LCSW) to provide holistic psychosocial support and therapy to patients in a home health setting. The clinician will assess resources, develop treatment plans, and coordinate with care teams to remove barriers to care.

The role emphasizes independence, technology-enabled care, and professional growth within a modern home health organization. AHCA Level 2 background screening may apply.

Qualifications

• Active, unencumbered Florida LCSW license.
• Minimum two years of clinical counseling experience (required).
• Home health or telehealth experience preferred.
• Valid driver's license, reliable transportation, and auto insurance.

Responsibilities

• Deliver holistic psychosocial support and behavioral health counseling/therapy to patients.
• Assess and coordinate community resources for patients with socioeconomic needs.
• Develop and implement treatment plans for patients with mental health conditions or other barriers to improved health outcomes.
• Communicate effectively with team members, providers, patients, and the community to ensure continuity of care.
• Provide excellent customer service while collaborating with patients, families, and team members in coordinating care.
• Maintain compliance with medical record standards, policies, and Medicare documentation guidelines.

Your Role as a Home Health Licensed Clinical Social Worker
