Job summary

Concierge Home Care in Florida is seeking a Licensed Clinical Social Worker (LCSW) for home health. You will provide psychosocial assessments, counseling, and coordinated care plans for patients in their homes, addressing social, emotional, and environmental barriers.

This role offers flexible scheduling, independence, access to AI-assisted EMR tools (WellSky/Kinnser), and supportive clinical leadership. Two years of clinical counseling experience and an active Florida LCSW license are required;

Qualifications

• Active, unencumbered Florida LCSW license.
• Minimum two years of clinical counseling experience.
• Home health or telehealth experience preferred.
• Valid driver’s license, reliable transportation, and auto insurance.

Responsibilities

• Provide psychosocial assessments, counseling, and behavioral health support.
• Develop and coordinate care plans addressing social, emotional, and environmental needs.
• Connect patients with community resources and support services.
• Assist patients and families in navigating social, financial, and healthcare challenges.
• Collaborate with physicians and interdisciplinary care teams to support patient outcomes.
• Educate patients and caregivers on coping strategies and available resources.
• Maintain documentation per Medicare guidelines.
• Document visits efficiently using AI-assisted EMR tools.

Your Role as a Home Health Licensed Clinical Social Worker

As a Licensed Clinical Social Worker (LCSW), you will provide psychosocial support and behavioral health counseling to patients receiving care in their homes. You will play a vital role in addressing social, emotional, and environmental barriers that impact health outcomes.
