Aviata Health Group in Bradenton, FL is seeking a full-time Licensed Clinical Social Worker (LCSW) or LMHC to join our family-focused care team in a Skilled Nursing and/or Assisted Living setting.

You will work with geriatric patients, collaborate with NPs, psychologists and other clinicians, and provide psychotherapy with a focus on quality of life. The role features weekday schedules, competitive pay, and comprehensive benefits, including PTO, holidays, and 401(k).

Formación

• Active and unrestricted LCSW in the state of Florida.
• Completed masters in social work.
• Experience with geriatric population is a plus.

Responsabilidades

• Provide psychotherapy to geriatric patients in a Skilled Nursing or Assisted Living setting.
• Collaborate with nurse practitioners, psychologists and other clinicians as part of an integrated care team.
• Support care planning and coordination for residents to improve quality of life.

As we continue to grow, we are looking for full-time licensed clinical social workers to work with the geriatric population in a Skilled Nursing and/or Assisted Living setting. Our psychotherapy include:

Qualifications
