Job summary

• Active and unrestricted LCSW in Florida.
• MSW degree required.

Responsibilities

• Provide psychotherapy to geriatric patients in SNF/AL settings.
• Collaborate with an integrated care team including NPs, PNPs, psychologists and social workers.
• Maintain patient records and coordinate with care plans.

Qualifications
