Job summary

Luminis Health, Inc. in Lanham, MD seeks a Licensed Clinical Social Worker to provide psychosocial assessments, individual/family/group therapy, and discharge planning for a patient caseload.

The role collaborates with the care team to implement identified services and ensure safe transitions of care, while engaging with families and significant others. The position requires MSW, an LCSW or LCSW-C license, and at least 1 year in mental health or substance use; two years in clinical social work

Qualifications

• Masters’ of Social Work degree (MSW).
• Minimum of 1 year of work in a mental health or substance use program.
• Masters degree in Social Work.
• Two years’ experience rendering clinical social work services in a health care environment.

Responsibilities

• Provides psychosocial assessments for patients on caseload and creates therapeutic framework.
• Provides therapy (individual, family, group) and addresses adjustment, trauma, and health-related issues.
• Identifies community services and links patients/families to resources.
• Collaborates with interdisciplinary team and communicates progress; revises plans as needed.
• Integrates medical/nursing information in assessments/interventions.
• Ensures timely evaluation and reporting of abuse/neglect; addresses domestic violence issues.
• Documents interventions in medical records per department standards.
• Facilitates patient/family support groups and mentors interns/staff.
