Luminis Health, Inc. is seeking a licensed social worker to provide psychosocial assessments and therapy across patient cases. You will collaborate with care teams to plan safe discharges and connect patients with community resources while documenting care in medical records.

The role requires an MSW and an active clinical license (LCSW/LCSW-C) with at least 1 year in a mental health or substance use setting. This on-site position involves working with an interdisciplinary team in Maryland.

Qualificações

• Master's of Social Work (MSW) required.
• Minimum of 1 year of work in a mental health or substance use program.
• Licensed Clinical Social Worker (LCSW) or LCSW-C required.

Responsabilidades

• Provide comprehensive psychosocial assessments for patients.
• In therapy role, develop a therapeutic framework for problem solving with patient and family.
• Collaborate with Care Coordinator on transition of care strategies in complex cases.
• Identify community resources and link patients to services.
• Collaborate with interdisciplinary team and communicate patient progress.
• Document interventions in the medical record.
• Promote psychosocial services within the care team.
• Participate in process improvement activities.
