Job summary

Central Florida Kidney Centers, Inc. is seeking a Full-time Licensed Clinical Social Worker based in Orlando to provide psychosocial support and concrete services to patients and families, and to assist with health care funding program follow-up.

The role requires an MSW from a CSWE-accredited program and an active Florida LCSW license, with specialization in clinical practice. The social worker will coordinate care, support discharge planning, and participate in QAPI initiatives within CFKC.

Qualifications

• Master's degree in social work from a CSWE-accredited program.
• Licensed Clinical Social Worker (LCSW) in the State of Florida.
• Specialization in clinical practice is required.

Responsibilities

• Composes and updates psychosocial evaluations and assessments; shares pertinent information with the care team.
• Participates in team reviews on patient care assessments and updates the care plan accordingly.
• Communicates with the discharge planner to ensure smooth transitions between hospital and outpatient services.
• Provides ongoing counseling to patients and families on emotional and lifestyle impacts of care.
• Maintains patient demographic data and documents in the EMR monthly.
• Identifies community resources and assists with applications for assistance and transportation logistics.
• Arranges and coordinates nursing home placements and home visits as needed.
• Ensures compliance with HIPAA policies and participates in QAPI and quality improvement efforts.
