Job summary

Nemours Children's Health in Orlando, FL is seeking a Social Worker (MSW) to join the dialysis care team, providing psychosocial support and care coordination for patients and families facing kidney disease.

The role focuses on collaboration with the healthcare team, comprehensive assessments, crisis intervention, and linkage to community resources. A CSWE-accredited MSW and 1 year of experience are required; 3+ years preferred.

Qualifications

• Master's Degree in Social Work from a CSWE-accredited program is required.
• Minimum one year of social work experience; 3+ years preferred (internship can count).

Responsibilities

• Provide comprehensive psychosocial services to patients admitted to Nemours Children's Hospital.
• Conduct biopsychosocial assessments and collaborate with interdisciplinary teams.
• Establish goals, reassess, and document patient/family histories.
• Provide crisis intervention services for domestic violence, psychiatric emergencies, or abuse.
• Assess mental health needs and refer for counseling/psychiatry as needed.
• Assist with admission screenings and psychosocial assessments; coordinate care.
• Offer case management, education, advocacy, and community referrals.
• Assist with child abuse investigations and manage domestic violence with medical teams.
