Job summary

Socket.dev is seeking a Social Services Director to lead the planning, development, organization, implementation, evaluation and direction of the facility's social service programs.

This role requires licensure as a clinical social worker and strong skills in policy development, discharge planning, community referrals and coordinating with agencies to meet residents' social and emotional needs.

Qualifications

• Minimum one year of experience in a human services field in a hospital, skilled nursing facility or related medical facility.
• Ability to organize and prioritize multiple tasks effectively.
• Skilled communicator, director and motivator.
• Reliable transportation required.

Responsibilities

• Plan, develop, organize, implement, evaluate and direct social service programs of the facility.
• Develop and implement policies for identifying medically-related social and emotional needs of patients/residents.
• Assist in discharge planning and provide referrals to community agencies when specialized help is needed.
